PCN Resource and Advocacy: Act as a PCN, departmental and organisational resource of expertise in dietetic/nutritional advice. Support the GP practices to refer patients to appropriate dietetic services across Manchester. Develop and sustain excellent working relationships with the practice teams within the PCN, working in a way that is collaborative and informed. Direct Clinical Work: Utilising specialist dietetic skills and knowledge to assess, plan, implement treatment and delivering dietetic interventions. Use specialist knowledge and critical appraisal skills to make clinical judgments in complex case management and formulates dietetic care based on best practice. DUTIES AND RESPONSIBILITIES Clinical trials 1. Check pharmacy files for missing data and organise, retrieve, and record data in a timely manner. 2. To support stock control within the dispensaries including ordering and receiving stock from Pharmacy stores and distribution and stock rotation. To process prescription and over the counter payments where applicable. To assist in ward stock top ups after appropriate training. This will involve: Visiting wards/departments and creating orders for stock medicines Filling the order Entering issues on the computer Ensuring wards/departments receive their stock (via porter/courier) in a timely manner. Put in place changes to reduce the prescribing of these medicines to high-risk patient groups. Management of medicines at discharge from hospital Reconcile medicines following discharge from hospitals, intermediate care and into care homes, including identifying and rectifying unexplained changes, and manage these changes without referral to a GP Perform clinical medication reviews, produce post discharge medicines care plans including dose titration and booking of follow up tests, and work with patients and community pharmacists to ensure patients receive the medicines they need post discharge. Set up and manage systems to ensure continuity of medicines supply to high-risk groups of patients (e.g. those with medicine compliance aids or those in care homes). To manage the environment and physical monitoring of the Aseptic unit maintaining appropriate records for microbiological cleanliness, pressures, room temperatures and equipment temperatures. To be responsible for performing daily cleaning of the Aseptic Unit. Key Result Areas & Performance: Technical Services To aseptically dispense sterile Cytotoxic products and other sterile injectables. To participate in the environmental and physical monitoring of the Technical Services Unit; maintaining appropriate records for microbiological cleanliness, pressures, room temperatures and equipment temperatures. To prepare aseptic product documentation and labels from prescriptions. To assemble aseptic products in preparation for aseptic processing. To assist in the dispensing of outsourced Parenteral Nutrition products. To ensure that products are prepared in an appropriate, timely and efficient manner. To accurately issue and transfer items used in Technical Services to appropriate cost centres using the Ascribe computer system. To be responsible for performing daily cleaning of the Aseptic Unit. To assist in the monthly cleaning of the Aseptic Unit. To perform in process volume checks, worksheet and label checks, and product assembly checks as required according to standard operating procedures. To supervise rotational staff within the Aseptic Unit, including pre-registration pharmacists and technicians, and assistants. To participate in the training of all staff new to the Aseptic Unit, including pre-reg pharmacists and technicians, and assistants in principles and procedures relating to aseptic dispensing and smooth running of the department. To assist in the day to day running of the Aseptic Unit as required by the section manager, to ensure workload is satisfactorily completed in order of priority. To assist in the updating of Standard Operating Procedures, information documents and instructions relevant to the Aseptic Unit, as requested by the Senior management. To participate in the production of Error Logs, QERs and reports relevant to the department as required by the departments standard operating procedures. To comply with the departmental Good Manufacturing Practice requirements, including dress and Hygiene relating to work in the unit. To work at all times within the Health and Safety Guidelines of the department. Maintenance of stock levels to ensure sufficient drugs and consumables are available for use within the unit. To be responsible for ensuring all laundry is packed and sent for processing. To undertake Level 2 NVQ Certificate in Pharmacy Services skills (QCF) within the required timeframe. To ensure accurate stock rotation and expiry date checks are carried out. To participate in stock takes. Ensure all waste is disposed of appropriately. General Participate in Dispensary weekend and bank holiday rotas as required. To ensure that the cleanliness and tidiness of the section meets specifications laid down by the section manager. To undertake personal continuing professional education and training as necessary to ensure high level of competency in carrying out the tasks required. To maintain safe systems of work at all times. To demonstrate good communication skills and practice good teamwork. Any other duties considered necessary to ensure smooth and efficient running of the department. Communications and Relationships Work closely with the Chief and Senior Technicians to ensure the smooth running of the Technical Service departments. Liaise with pharmacists, technicians and other staff within the Pharmacy department as required. Communicate with staff from wards and departments around the hospital as required.
