To be responsible for the delivery of a countywide specialist learning disability Physiotherapy service ensuring that the service provided is in line with the CQC regulatory requirements. To provide professional leadership & management to the Physiotherapy staff (and students) including recruitment, supervision, appraisal and professional development particularly mandatory training and education. To provide specialist Physiotherapy input into planning, development, and evaluation of learning disability services, holding responsibility for defined projects as delegated by the LDP management team. As an autonomous practitioner to be responsible for and manage a highly complex case load, including planning and implementing appropriate clinical interventions based on the referral evaluation, assessment, diagnoses, treatment and monitoring of individuals identified needs for adults with a learning disability. To lead in Multi-disciplinary/ multi-agency working within the LDP and with Key partners, to keep the patient at the centre of care, ensuring patient and carer participation in decision making. To provide professional leadership and management responsibility within the LDP as part of the extended Management Team.

To co-lead focus groups and consultations with practitioners across NHS and third sector services (e.g., health visitors, clinical psychologists, psychotherapists, occupational therapists, social workers) to gain insights through sharing experiences of evidence-based practice and integrating learning. To co-lead focus groups and consultations with caregivers of young children in public and patient involvement initiatives that aim to co-produce guidance for cross-service use. To exercise autonomous professional responsibility for project outputs (e.g., service frameworks, literature reviews, reports, and guidance) in line with service guidelines and under the overall co-ordination of the Principal Investigators. To communicate, in a skilled and sensitive manner, to other practitioners, service users, and other key stakeholders as appropriate. To attend and contribute to team meetings and discussions. To work in partnership with other disciplines and to maintain links with Statutory and non-statutory and primary care agencies as appropriate. To contribute to the reduction of risk in areas of responsibility. To produce project reports and notes in a timely manner and to provide management information in line with Trust policies and procedures in a timely manner.

As the LDA H@H OT in the team, you will work as part of the multi-disciplinary team to assess, plan, implement and evaluate the specialist health care needs of children and young people who have a learning disability and/ or Autism. The team will have a phased implementation: During Phase 1; the senior team will be offering consultations to the network and contributing to individual cases. They will be preparing for the training and preparation for the rest of the team. During Phase 2; you will be part of discussions for planned implementation of agreed interventions, on a small number of agreed pilot cases. You will be part of the team to put the agreed interventions in place, working collaboratively with the young person, family and other relevant professionals in the network for that individual. You will have an identified supervisor and attend both 1-1 and group supervision/ debriefs for these complex cases. During Phase 3; You will continue to be part of the MDT as the team expands, continuing to contribute to the team process' and development.
