To provide reports as requested to support other processes within the prison including e.g. home detention curfew, release on temporary licence, MAPPA and parole, and to attend case review boards as appropriate. To work collaboratively and effectively with social workers, probation officers, healthcare professionals and third sector colleagues (whether employed by Pact or other organisations) in the community in line with family-focused assessments, plans and casework reviews. 11. Practicing at a Highly Advanced level in a specialism within the directorate, our directorate lead role balances leadership and clinical practice to deliver compassionate excellence. Reporting to the Divisional Lead Pharmacist, they maintain a key relationship between their clinical directorate and the pharmacy directorate to ensure good medicine management and the delivery of patient focussed medicines optimisation. The Directorate Lead Pharmacists act as role models in terms of Trust values and behaviours as well as clinical expertise. The post-holder will lead, develop, deliver, evaluate and be responsible for delivering a comprehensive pharmacy service and medicines optimisation advice to the directorate. The T&O directorate includes Trauma (JR site), Orthopaedics (Nuffield Orthopaedic Centre - NOC site) and Rheumatology (NOC site) and the role includes line management an expanding team of pharmacists responsible for day to day clinical cover for these areas. Works in collaboration with services across the cancer/and other disease pathways, to implement plans and to monitor their effectiveness in improving service delivery and meeting the needs of people living with cancer and other palliative diagnosis. Leads by example, contributes to the delivery of specialist education and training to other professionals involved in patient care; promotes best practice; supports and coaches less experienced colleagues to achieve high quality, personalised care for people living with cancer and other life limiting illnesses. Contribute to the hospice education program liaising with the Education and Professional Development Managers to ensure this is current and addresses the needs of our own staff and those who work in external teams and organisations Provide mentorship and support when required to junior staff, staff undertaking the hospice development program and student nurses allocated to the hospice from our local partner Higher Education Institutes ensuring timely assessment and completion of clinical competencies. Clinical Excellence Delivers personalised care through regular supported conversations with the person living with a palliative diagnosis, identifying needs through a holistic needs assessment, and addressing patients health care needs. Following a holistic needs assessment, develops a comprehensive care and support plan with the person living with a terminal illness. Reviews and updates the plan every time a supportive conversation takes place, which should be regularly and evaluates the impact of care interventions. Actively integrates theory and practice. Act as a key worker for a clinical caseload, working to coordinate care and provide easy access for ongoing support for patients. Participates in a 7 day service as part of a rota. Assesses peoples information needs and provides information, to determine their own future. Enables people to access, identify, appraise and interpret complex technical information to help them to determine their own future. Provides clinical expertise, specialist advice and support across service boundaries. Leads the nursing contribution to service development, ensuring the pursuit of excellence in care. Defines a clear care pathway, refers to other professionals and signposts to other agencies as appropriate. Promotes the service and clinical excellence by contributing to publishing and presenting innovations, audit and research locally, regionally and nationally. Influences and develops policy and clinical guidelines internally and externally. Promotes self-management, risk stratifying patients into who can self-manage following education, those who will need guided support to self-manage and those who will require ongoing, face-to-face support, to support people to live with their diagnosis and the choices available to them.

Carries out nursing duties within the Trust Policies and Code of Professional Conduct. Carries out instructions from the Ward Manager and other Senior Staff. Carries out treatments as requested from Multi-Disciplinary Team. Under the control of the Ward Manager teaches and monitors nurses allocated to the Ward. Adheres to the agreed procedures as laid down and promotes, by example, a high standard of nursing care in the Ward. Fosters good relationships with other departments, the Multi-Disciplinary Team, patients, relatives and visitors to the Ward. Helps achieve high moral for both staff and patients. Must be aware of relevant ordering and stock control, management practices, policies and any other aspects of nursing and understand their responsibilities. Ensure all written documentation is of a high standard, accurate and kept up-to-date according to NMC regulations. Becomes aware of responsibilities under the Health and Safety at Work Act, reports on any practices that could be detrimental to patients and staff safety. Follow COSHH regulations. Attend statutory training yearly for Fire, CRP, Lifting/Handling and Equipment. Professional: Responsible as a trained nurse for the control and administration of drugs within the regulations laid down by the Bradford Hospitals NHS Foundation Trust and the NMC. Develop skills and knowledge in Clinical Practice. Maintain professional update. Work within the NMC Code of Practice. Maintain personal, professional profile. Develop within the Scope of Professional Practice. Achieve PREP as dictated by NMC regulations. Practice agreed models of nursing and systems of care as directed by Ward Manager.
